What is Angioplasty Surgery Cost in India?

Coronary artery disease (CAD) is a prevalent heart condition and one of the significant causes of death worldwide. In this, arteries get blocked due to cholesterol plaque buildup, and consequently, the blood supply to the heart muscle is restricted. A solution to this heart condition is angioplasty. It is a minimally invasive treatment procedure performed to reopen blocked arteries and restore blood flow. It can be done with or without the application of a stent.

The average cost of angioplasty in India with one stent  ranges between INR 200,000 and INR 3,00,000 for Indian patients. For international patients, the cost ranges between USD 4500 and USD 5500. What is Angioplasty Surgery?

In the past decade, coronary heart disease (CAD) has appeared as a significant cause of death. With an estimated prevalence rate ranging between 1.6% to 7.4% in rural populations and from 1% to 13.2% in urban populations, it's becoming a matter of health concern. Some of the common symptoms associated with CAD are listed below.

In CAD, cholesterol and fat build-up (collectively referred to as plaques) in the coronary artery. This leads to blocked or interrupted blood flow in the heart. Sometimes, those deposits can break suddenly and form a clot that leads to a heart attack or stroke. This fatal outcome justifies the importance of immediate medical intervention.

To treat these conditions associated with blocked coronary arteries, a percutaneous coronary intervention (PCI) known as angioplasty is performed. It involves inserting a thin, flexible tube called a catheter into the blocked blood vessel. At the tip of the catheter, a deflated balloon is attached. 

Once the catheter reaches the narrowed area of the blood vessel, the balloon is inflated to compress the plaque against the artery walls, thus widening the vessel and restoring blood flow.

In some cases, a stent, a small mesh tube, may be placed in the artery to help keep it open. After the procedure, the balloon is deflated, and the catheter is removed. However, the stent is kept in the same place to prevent restenosis.

Angioplasty typically focuses on restoring blood flow in arteries. It is often an emergency procedure during a heart attack. It is mainly influenced by risk factors such as increasing age, sex (more prevalent in women), diabetes, high blood pressure, and tobacco consumption.

Balloon Angioplasty

It involves inflating a tiny balloon within the narrowed artery to widen it and restore blood flow. In advanced procedures, drug-eluting balloons (DEB) are used to deliver medication directly to the affected area. When inflated at the site of the blockage during angioplasty, the DEB releases the medication into the arterial wall. This drug inhibits the excessive growth of cells that contribute to restenosis.

Stent Placement

After balloon angioplasty, a small mesh tube called a stent is often inserted to support the artery and prevent it from narrowing again. 

The tissues in the heart will start coating the stent like a layer of skin, and within 3-12 months, the stent will be fully covered by the skin. Based on their properties, stents can be classified as:

  1. Bare metal stents (BMS): A stent is made of thin, uncoated (bare) metal wire that has been formed into a mesh-like tube.
  2. Drug-eluting stents (DES): Similar to regular stents, DES are coated with medicines that prevent arterial narrowing that may occur after stent implantation.
  3. Bioresorbable scaffold system (BRS): It functions as a temporary support structure for the affected artery. Once the blockage within the vessel is successfully treated and healed, this stent dissolves completely, leaving the artery free from any traces of the stent material.

Cutting Balloon Angioplasty

It uses a balloon with tiny blades to create longitudinal incisions and expand specific resistant plaques that are hard to push back.

Atherectomy

In this, a catheter is inserted by making a small incision. On its opposite end, a sharp blade, laser, or rotating device is connected. With the help of its opposite end, plaque is then scraped away, dissolved, or broken up without making a large incision.

Yes. Most people take 2-3 weeks to start returning to their normal activities after angioplasty/stenting. However, it's essential to follow your doctor's recommendations for recovery, which may include medications, lifestyle changes, and regular follow-up appointments.

Cardiac surgeons frequently recommend angioplasty in case of a sudden heart attack. This procedure diminishes the possibility of subsequent complications, such as heart failure. Moreover, angioplasty typically requires only 1-3 hours to perform and is the safest and most effective method for restoring blood flow to the heart, thus preventing any potential fatal outcomes.

The chances of severe complications are rare. Sometimes, damage to the artery where the sheath or catheter were inserted, allergic reaction due to foreign material, or excessive bleeding may occur. These can be controlled by careful monitoring and follow-ups.

Stents last a lifetime. Once they have been expanded within the artery and attached to the blood vessel, they will not move. The artery can block again within the stent, but the stent itself remains where it was placed during angioplasty.

In most modern angioplasty, a short wire mesh tube called a stent is inserted into the artery during the procedure and left there permanently to allow uninterrupted blood flow. This is mainly used to prevent restenosis.

There are two main types: bare metal stent (BMS) and drug-eluting stent (DES), which release medication into the artery to reduce scar tissue formation and prevent narrowing. Both DESs and BMSs are meant to be permanent. In some cases, it was found that DESs are less likely to re-narrow and have fewer complications as compared to BMSs.

Stenting does not remove the blockage. It only props the artery open by displacing the blockage outwards to provide blood to the heart muscle. Hence, there are some chances of recurrence, and to reduce it, continuous medications and adapting to a healthy lifestyle are recommended.
