Dr. Sandhu discussed Complex Coronary Interventions, including Rotabalator, Intravascular lithotripsy (IVL), Endovascular procedures, including Endovascular Aortic Aneurysm Repair (EVAR) or Thoracic Endovascular Aortic Aneurysm Repair (TEVAR), Angioplasty, etc.
He mentioned that 80% of Coronary Artery Bypass Graft (CABG) are simple blockages that can be sorted with Angioplasty.
The patients undergoing Angioplasty or CABG can be discharged the very next day, and in the case of an international patient, they can fly back within a week.
CABG uses different sources of grafts, including:
- Veins (specifically saphenous)
- Arteries (mammary and radial arteries)
Dr. Sandhu also elaborated on IVL, in which miniaturized and arrayed lithotripsy emitters create a localized field effect at the calcium site.
Shockwave IVL procedure:
- The IVL Catheter is used to cross a calcified lesion.
- The incorporated balloon is inflated to aid in effectively transferring energy.
- The emitters release an electrical discharge that vaporizes the fluid inside the balloon.
- This creates a rapidly expanding & collapsing bubble that generates sonic pressure waves.
- The localized field effect created by the waves travels through soft vascular tissue, selectively cracking the calcium within the vessel wall.
- After calcium modification, the integrated balloon may dilate the lesion at low pressure to maximize luminal gain.
