Bilateral ptosis is the hallmark of involvement of the central caudal nucleus, which regulates the levator palpebrae superioris muscle on both sides. Ptosis is caused by the weakness of the elevator muscles of eyelids, either levator palpebrae superioris (LPS) or muller muscle. Levator dysfunction is caused by muscle, nerve, or nucleus involvement.
