Sinusitis is of different subtypes and all require different scope of medical or surgical treatment. Allergy such as sneezing or watery nasal discharge is seasonal and can always come back depending on immunity and environmental issues. Outcomes of endoscopic sinus surgery depend upon the subtype of sinus disease (polyp vs non-polyp sinusitis), kind of surgery performed (polypectomy vs endoscopic sinus surgery), extent of surgery performed (basic vs extended), presence of bronchial asthma or aspirin hypersensitivity and clinical and endoscopic follow-up post-surgery.
Antibiotics have very less penetration in bony sinuses inflamed over long periods and coupled with resistant bacterial biofilms, antibiotics have a very limited role to play in management of chronic sinusitis. Surgical disease removal followed by nasal saline and medicated washes are the most effective method for removing inflammation and infection. Further, frequent antibiotic courses lead to antibiotic resistance not just in the patient but also in the community and can have potential adverse effects on immunity.
Functional endoscopic sinus surgery is a minimum invasive surgery which is used for restoring the sinus ventilation and its normal functioning. This procedure is aimed to restore the sinus ventilation and its normal functions.
Most of the candidates for this surgery are those who have recurrent acute or chronic infective sinusitis and they are looking for improvement in symptoms of 90 percent during the procedure. This procedure is also mainly used for those patients whose medical treatment has failed.
Endoscopic surgery is called functional because it is used for distinguishing endoscopic from non endoscopic. The aim of the surgery is to return the mucociliary drainage of the sinus back to normal function.
FESS is considered as a very safe surgery because it has got very few risks and complications. Only 5% complications has been reported with FESS surgery.
FESS is not a major surgery it is a minimum invasive surgery which is being used in removing sinus polyps and other type of abnormalities of the nose which can cause significant breathing problems.

Functional endoscopic surgery is being done either under general or local anaesthesia and it takes around 30 to 90 minutes to be done.
Many patients with sinus infections doesn’t need any surgery, medical treatment and lifestyle changes usually keeps the symptoms under control but if those changes doesn’t work then surgery is the best alternative for it.
In FESS surgery, the surgeon cuts directly into the outside of the face for detecting the problem and removing it. Once the procedure is finished the temporary nasal package will be placed in your nose to support the newly opened sinus to absorb excess fluid while you tissues heal.
It can take several weeks for you to fully recover. You'll have some swelling and tenderness inside your nose after the surgery, but this is normal. You may have symptoms like a severe cold or a sinus infection.
You should expect some nasal and sinus pressure and pain for the first several days after surgery but your nasal passage and breathing should return to normal within 2-3 weeks after surgery.
Use salt water (saline) rinses beginning 1 to 2 days after surgery to wash away any crust and surgical debris and your doctor may recommend that you use a nasal steroid spray 15-30 minutes after using the salt-water rinse and you will need several visits after surgery to clear out old blood and mucus.
You will be able to return to work in one week and do your normal routine about three weeks but it varies according to the job and the extent of the surgery. Most people are able to feel normal within one to two months after surgery, you will have to visit doctor regularly for three to four months after surgery.
Recovery time after surgery depends on the overall health, you have to follow some instructions which your doctor has given to help in your speedy recovery.
Every patient tolerates pain differently. Most patients manage sinus surgery pain with oral pain pills. Your doctor can tell you what to expect, based on the type of surgery you have. Facial swelling or bruising is rare, but the inside of your nose will be swollen and sore for a while. This may make breathing through your nose difficult for about two weeks.
