Caldwell- Luc antrostomy surgery is the surgical treatment for treating severely diseases maxillary sinus. This is also an alternative to middle meatal antrostomy which is being done by endoscopic surgery. This surgery was the primary approach for treating the maxillary sinus before the beginning of endoscopic sinus surgery.
Caldwell- Luc antrostomy is indicated in case of complicate acute or chronic rhinosinusitis, pterygomaxillary space surgery, trauma, foreign body retrieval, benign tumor removal, orbital decompression and inverting papilloma removal
This procedure is contraindicated in children as there as chances to damage to their secondary dentition.
The surgery takes about 1 to 3 hours under general anaesthesia depending on the condition of the patient.
Caldwell- luc procedure the incision is made from lateral incisors to the second molar, then mucosal and the periosteal flap is elevated and dissected to expose anterior wall of sinus. At last the anterior wall is opened in canine fossa where bone is thin with the drill.
This surgery is done under general anaesthesia. Incision are made in the gingivobuccal sulcus above canine fossa which is continued through the periosteum. A window is cut into the maxillary antrum with osteotome, with the help of punch forceps the windows are enlarged. Under inferior nasal concha the curved hemostat is passed into the sinus. Maxillary sinus can be visulaized through both the windows. At the end the gingival incisions are being closed.
Most common complications of Caldwell-Luc antrostomy are facial swelling or numbness, facial and cheek discomfort, facial asymmetry, facial paresthesia, hemorhage, oro-antral fistuals, gingival labial fistuals etc.
You will have to sleep with your head elevated for 24-48 hours after surgery to help the bleeding slow down and help the swelling resolve.
The patient should be advised against obsessive blowing of the nose for at least a week.
You can return back to your normal activities in a week after surgery but it may take upto three week. Complete recovery will take about one to two month, you have to visit your doctor regularly after three to four months of surgery.
