Before the Procedure (5 Questions):

Biventricular pacemaker is an implantable device which is being used in those people who are having advance heart failure because of abnormal rhythm and function of heart. Biventricular pacing system consists of three wire leads and a pulse generator which has a battery and a small computer.

Biventricular system  works to deliver electrical stimulus to prevent the heart rate from becoming slow and the third wire should stimulate both the left and right ventricles of heart to co-ordinate with beating of heart.

For proper functioning of healthy heart the electrical signals will help your left and right chamber of heart to pump together. When heart failure occurs the abnormal signals causes contraction of ventricle to fall out of synch which can also be due to  lack of coordination with atria. This reduced the number of effective contraction and the amount of oxygen rich blood pumped out the body will make you feel sick.

When there will be an abnormal rhythm the painless electrical impulses will be generated which will pass through the leads to the heart chamber and help them in pumping together. Lead tips are advanced until they make contact with your right atrium, right ventricle and left ventricle. The pulse generator is inserted through the chest incision and held in place by a pocket of nearby tissue. You will not feel pacing of your heart.

Biventricular pacemaker is usually needed for those people who have abnormally large left ventricle, poor ejection fraction, life threatening arrhythmia and severe or moderately heart failure symptoms such as fatigue and shortness of breath.

During the Procedure (3 Questions):

It will take around two to five hour to implant biventricular pacemaker.

Biventricular pacemaker is being done under general anaesthesia. First a  cut is made below the collarbone and then the doctor puts the pacemaker which grows through the cut and into the large blood vessel and then in the heart.

Few surgical approaches to implant biventricular devices are:

  • Endocardial (transvenous) approach: This approach is minimally invasive. A small incision is being made in your arm region which is  nearby then advanced to the nearby blood vessel and they reach the heart.
  • Epicardial approach: This approach is open surgery that involves making a larger incision near your heart. This approach involves making larger incision in your heart and this approach is suitable for those patient who needs multiple cardiac treatment in single procedure.

Post the Procedure (7 Questions):

Once the leads are in place, your healthcare provider performs lead function testing. This is also known as pacing which confirms that where the device should be, they should function properly and they are capable of synchronizing left and right ventricle activity. Further adjustment is made by adjusting the device which is made by placing a programmer over the site of generator.

A biventricular pacemaker improves symptoms of heart failure in about 50% of people who receive the device. Better quality of life and ability to live an active, independent lifestyle, Fewer heart-related hospitalizations and improves heart functioning and improves survival rate.

Some of the risks associated with biventricular pacemaker are bleeding, filling of the space near the heart with blood, collapsed lung, infection, dislodgement of lead and mechanical failure.

The stay in hospital depends on the recovery, you can go home within one day to five days after the procedure.

Recovery can take a few months. You should rest and avoid strenuous activity involving your upper body during this time.

If treatment is successful, you may experience  gradual improvement in symptoms to most daily activities, Improved ejection fraction, normal or near-normal sized left ventricle and lower risk of cardiac arrest.

If you experience some symptoms such as abnormal bleeding from incision site, pain which become worse and do not respond to medications, redness, warmth or swelling at incision site, pericardial effusion , severe bruising and symptoms of infection such as fever or chills.
