Before the Procedure (17 Questions):

In an allogenic transplant, stem cells are collected from a matching donor and transplanted into the patient to suppress the disease and restore the patient's immune system. An allogeneic stem cell transplant is different from an autologous stem cell transplant, which uses stem cells from the patient's own body.

An autologous stem cell transplant uses the patient's own stem cells, harvested, frozen and then infused back into the body after chemotherapy or radiation therapy. An allogenic stem cell transplant uses stem cells collected from a donor including a sibling with perfectly matched stem cells.

It takes about 24 hours for your stem cells to reach the bone marrow. Then they start to grow, multiply, and help the marrow make healthy blood cells again.

Allogeneic stem cell transplantation involves transferring the stem cells from a healthy person (the donor) to the patient's body after high intensity chemotherapy or radiation.

In most cases, the donor is given medicine so the bone marrow comes to the blood flow. One unit of the blood is retrieved from the donor. The bone marrow is then taken out of the blood of the donor. So essentially, for donor the procedure nothing more than donating one unit of the blood.

If the patient has siblings, there are 30% chances that the sibling is fully matched HLA. However,in the absence of siblings OR unmatched siblings, the doctor will search for the matching HLA in the worldwide registry. There are 30% chances that you may find a fully matched unrelated donor somewhere in the world. If neither of these works then the parent of the patient can become a donor but they will be half-matched donor.. Which is called haplo transplant OR half-matched donor transplant. The success rate of a fully matched sibling transplants is about 90%. A fully matched unrelated donor is also about 80-90%. For half-matched transplants through the parents, the success rates are between 60-70%.

Patients stay at the hospital anywhere from 30 to 50 days after they receive their transplant. Post-discharge, the patient is advised to stay nearby for another 90 days at least.

Success rates are very high in the case of Fully Matched Unrelated donor. So if you are fortunate to have found a Fully matched unrelated donor in the worldwide registry, you should choose it over half match (Haplo) transplant.

Before a person receives an ALLO transplant, a matching donor must be found using human leukocyte antigen (HLA) typing. This special blood test analyzes HLAs, which are specific proteins on the surface of white blood cells and other cells that make each person's tissue type unique.

Allogenic bone marrow transplant is the procedure in which the healthy blood stem cells from the donor are used to replace the diseased or the damaged bone marrow.

An allogenic bone marrow transplant is the treatment option of people who are having different types of cancerous and non cancerous diseases such as acute leukemis, aplastic anemia, chronic leukemia, Hodgkin’s lymphoma, immune deficiency, multiple myeloma, neurblastoma, non-hodgkin’s lymphoma.

Autologus means self the stem cells in the autologus transplant will come from the same person who get transplant so the patient is their own donor. While allogenic means other the stem cells in the allogenic transplant are from the person other than the patient they can either be the match related or an unrelated donor.

During the procedure of bone marrow transplant patient does not experience much pain first a small incision is being made through which a wide bore needle is being inserted and the stem cells are being collected. You may experience pain after the anesthesia wears off, pain killers are needed for few days after surgery.

The life expectancy of the patient has increase significantly after a bone marrow transplant, the mortality rate of the patient has been increased significantly after bone marrow transplant.

In allogenic bone marrow transplant the stem cells are being replaced with the new healthy stem cells. The cells are from the donor or from the donated umbilical cord blood, before the transplant chemotherapy or a combination of chemo and radiotherapy are given.

One of the benefits of allogeneic stem cell transplantation is that after the donated cells engraft in the patient, they create a new immune system.

During the Procedure (6 Questions):

Allogenic bone marrow transplant will take around one to two hour to complete. It may be possible that you may feel some soreness or weakness for few days after the procedure.

Allogenic bone marrow transplant is being done by a hematologist or an oncologist.

In case of allogenic bone marrow transplant the stem cells from the person are being replaced with the healthy and new stem cells from the donor.  Before the implant the patient is given a combination of chemotherapy and radiotherapy.

The procedure takes 1 or 2 hours. You can go home that day or the next morning.

Allogenic bone marrow transplant blood stem cells or bone marrow will be infused one to three days after the last chemotherapy or radiation dose. The days after the transplant are being counted.

It can take 6 to 12 months, or even longer, for blood counts to get close to normal and your immune system to work well. During this time, your team will still be closely watching you. Some problems might show up as much as a year or more after the stem cells were infused.

Post the Procedure (6 Questions):

It will take around two to six weeks to recover completely from allogenic bone marrow transplant. This is time when you start seeing your blood count return to the normal level.

Bacterial infections are the most common complication of allogenic bone marrow transplant. Some of the other complications of the procedure include viral, fungal and other infections can also occur.  Some infections can develop later on, weeks to months after the transplant.

The survival rate of the patient after allogenic bone marrow transplant is approximately 98. 5%.

The survival rate after allogenic bone marrow transplant is 10 years, the risk of mortality remains higher than the age.

After allogenic stem cell transplant you need to take precautions such as avoiding the source of infection, washing your hands regularly and following a food safe diet. Before an allogeneic stem cell transplantation, the patient receives a conditioning regimen of chemotherapy and, sometimes, radiation therapy. This conditioning treatment is given to destroy any remaining cancer cells in the body.

One complication of allogeneic transplantation is that the patient’s body may reject the donated stem cells before they are able to engraft in the bone marrow. Another complication of allogeneic transplantation is that the immune cells from the donor may attack healthy cells in the patient’s body. The parts of the body that are most commonly damaged by GVHD are the skin, intestines, liver, muscles, joints and eyes.
