One should apply for an Indian medical visa only when his/her sole purpose of visiting the country is medical treatment.

Hospitals consulted in India by the applicants for their treatment should be reputed/recognized/specialized.

Important Documents Required for Indian Medical Visa from Azerbaijan

Correctly filled online application form containing complete signatures.

Passport: both original and photocopy.

Providing return tickets is mandatory.

Proof of availability of sufficient funds.

2 current color photographs against a white background of size 51mm x 51mm.

Medical reports of the patient.

An official letter from the local doctor in Jordan expressing the need for a specialized treatment for the patient.

A visa invitation letter from the concerned hospital in India mentioning the patient and his/her medical attendant’s name, treatment, number of days required for the treatment.
Online Application of Indian Medical Visa from Azerbaijan

Click on Regular Visa Application at the Indian Visa Online website.

Enter details about the name of your country, high commission, your date of birth, nationality, e-mail id, expected date of arrival in India and visa type. Click Continue after entering the access code.

Fill up each of the three pages of the form with the correct details and click on Save and Continue at the bottom of each page.

Put your family address and your profession or occupation.

Enter the details regarding your medical visa application that includes: the hospital in your country, the concerned hospital in India, time taken for the treatment, no. of entries, reason of visiting the country, expected date of journey, estimated arrival and departure time in India, other information, details about SAARC country visit and reference.

In case you do not have a reference in India, you can provide Vaidam’s contact details, only if you travel through the company’s assistance.

Upload a digital photograph in the form following the below listed parameters:

It should be of JPEG format.

The size of the photo should not exceed 300kb. Minimum size should be of 10kb.

The photo should have equal height and width.
-
The head should be at the center of the frame, full view (from top of hair to bottom of chin), front face, eyes open.

The head should measure 25 mm to 35 mm. The eye height should be 1 to 1/8 inches – 1-3 inches.

The background should be either white or light colored.
-
There should be no shades on the face or the background.

The photo should not have any borders.

A preview of the online visa application form will appear in a new window once you click Save and Continue on the third page.

Go through them thoroughly. Click Verified and Continue if every data entered by you is correct. In case of rectifications, click on Modify/Edit.

Read the set of instructions that appear in another new window and click OK for final registration. For any changes, click Cancel.

Select an appointment date for the submission of medical visa application from the mission counter in the new window and click on Confirm the Appointment.

On doing so, in a new window, you will get two options: Save and Print. Save the online visa application.

Click on Print Registered Application to get a print of the visa application form.

Put your full signature inside the rectangular box located just below the photo box. Do the same in the second page as well. The signature box is located at the bottom.

Submit the application form along with the necessary documents at the Mission Counter on the appointed date. The date is mentioned at the right margin of the application.
Indian Medical Visa Processing Fees
The processing fee of Indian medical visa from Azerbaijan is 147 AZN for a duration of up to 6 months and 219 for up to 1 year.
Note: Medical visa fees are accepted in cash and in AZN currency only.

Indian Medical Visa Processing Time
3-4 working days are required for processing Indian medical visa for Azerbaijan nationals. For non-residents of Azerbaijan, the medical visa processing time is minimum 5 days.

Medical visa Information after Your Arrival in India
Medical Visa Extension
Medical visa requires extension when the patient requires some more days for the completion of his treatment or his/her complete recovery. In order to extend ones medical visa, he/she would require an official letter from the concerned hospital authorities stating his/her medical condition, treatment procedure and the number of days extra that will be required for his/her complete recovery. The patient or the medical attendant accompanying him/her then has to go to the FRRO office along with the letter issued by the hospital and other required documents to apply for a medical visa extension.
FRRO
Foreign Regional Registration Office (FRRO) deals with and administers the registration, departure, visa extension and other official records regarding foreigners’ arrival and stay in the country. Once patients from abroad arrive in India, they have to get themselves registered in the FRRO office. The documents required for the registration is listed below:

E-Tourist Visa from Azerbaijan to India
E-tourist visa can be used for medical travel when patients require short term medical treatments or check-ups. Here are the documents that are required for E-tourist visa.

The size of the PDF should be 10KB to 300KB.

A digital photograph is to be uploaded. The photograph should maintain the following criteria:

The height and width of the photo should be equal.

Photo should have full face, front view and eyes open.

Center head within the frame. Head from top of the hair to the bottom of the chin of the individual should be prominent.
