Thrombolysis
Also referred to as thrombolytic therapy, it is a treatment that dissolves dangerous clots in blood vessels, improves blood flow, and prevents damage to tissues and organs. It involves the injection of clot-busting drugs through an intravenous line or through a catheter that delivers drugs directly to the site of the blockage. It also uses a long catheter with a mechanical device attached to the tip that removes the clot or physically breaks it up. Thrombolysis is mostly an emergency treatment to dissolve blood clots formed in arteries feeding the heart and brain. These are also the main cause behind heart attacks and ischemic strokes and in the arteries of the lungs.
Types of Thrombolysis
The most commonly used clot-busting drugs are:
· t-PA (class of drugs that includes Activase)
· TNKase (tenecteplase)
· Abbokinase, Kinlytic (urokinase)
· Emina's (anistreplase)
· Retavase (reteplase)
· Striptease (streptokinase)
The doctor may choose to inject clot-busting drugs into the access site through a catheter depending on the circumstances.
Risks of Thrombolysis
Thrombolysis can safely and effectively improve blood flow. It can relieve or eliminate symptoms in many patients without invasive surgery. But it's not recommended for everyone. Thrombolysis may not be suitable for patients who use blood-thinning medication, herbs, or dietary supplements. It is not recommended for people with certain conditions associated with an increased risk of bleeding. These conditions include:
· Severe high blood pressure
· Severe kidney disease
· Recent surgery
· Active bleeding or severe blood loss
· Haemorrhagic stroke from bleeding in the brain
Thrombolysis also may be very risky for patients who are pregnant, at an advanced age, and people with other conditions.
Patients who undergo thrombolysis have a lesser risk of infection as well as a little risk of an allergic reaction to the contrast dye that may be needed for imaging.
Other possible risks include:
· Bruising or bleeding at the access site
· Damage to the blood vessel
· Migration of the blood clot to some other part of the vascular system
· Kidney damage in patients with diabetes or another pre-existing kidney disease
