Ovarian Tissue Freezing or Cryopreservation
Women and girls who are undergoing some medical treatment like cancer surgery, radiation, chemotherapy, may experience impaired fertility or hormone production. Individuals who undergo gender affirmation surgery also wish to preserve ovarian tissue.
This process is a boon for them as they can get the egg-producing portion of the ovary, called the ovarian cortex removed before a patient’s treatment. It is then frozen and stored. Years later also these tissues can be transplanted to make pregnancy possible.
Ovarian tissue preservation also has the additional advantage of hormone production. Damaged ovaries cannot make important hormones like estrogen or progesterone. To restore natural hormone production previously frozen, healthy ovarian tissue can be reimplanted after treatment.
The Procedure
While the patient is asleep under anesthesia, the surgeon makes a small incision in the abdomen to insert a narrow tube called a laparoscope. The surgeon then removes tissue from the surface of one ovary that contains immature eggs. One ovary is removed to harvest an adequate number of tissues. The tissue is then divided into small pieces, and flash frozen. Frozen ovarian tissue are often kept in specially equipped storage facilities for several years where they're carefully monitored and preserved.
After completion of treatment, the one that had ovarian tissue removed and frozen is prepared to possess ovaries restored. Doctors can now thaw the tissue and return it to the body. Laparoscopy is used for this procedure. The surgeon may form a pocket within the peritoneum, a membrane that surrounds the organs within the abdomen, and deposit the tissue into the pelvis on the brink of the remaining ovarian tissue.
After the thawed tissue is replaced, ovarian activity should begin or resume after a couple of months. At this stage pregnancy attempts can be initiated under the guidance of a fertility specialist.
Risks of Freezing Ovarian Tissue
Harvesting the tissue and transplanting are both surgical procedures that are performed with anesthesia. Using anesthesia is a risk in itself. For patients affected by blood cancer there is a risk of reoccurrence of cancerous cells into the body during the time the ovarian tissue is reimplanted.
Conclusion
If the transplantation is successful and therefore the patient started producing eggs, pregnancy can occur naturally or through in vitro fertilization (IVF). The procedure leads to successful pregnancy in about one-third of patients. For further details, you can contact one of the best Gynaecologists in Ampang Malaysia, Dr. Suhaimi Bin Isa.
