Myelodysplastic Syndrome is a rare disease in which the production of the blood cells in the bone marrow is disrupted causing more immature and defective cells than mature, healthy ones. It is considered a rare type of cancer. 

The proven cure for Myelodysplastic Syndrome is stem cell transplant. Also called bone marrow transplant, doses of chemotherapy drugs and/or radiation are given to clear out the defective cells and replace them with healthy, donated (allogenic) stem cells.  Other options for treatment include blood transfusion and drug therapy.

Symptoms of Myelodysplastic Syndrome

Usually, people over 60 years of age acquire Myelodysplastic Syndrome. Although it can happen to younger people as well. It could range from mild to severe and depends on person to person.

Symptoms are not found in the early stages and are sometimes found during a routine blood test. Symptoms include:

  • Shortness of breath
  • Fatigue
  • Paleness due to anemia
  • Unusual or easy bleeding or bruising 
  • Tiny red marks under the skin
  • Frequent infections
  • Bone pain

Risk factors of Myelodysplastic Syndrome

  • Older age: 60 years and above
  • Chemotherapy or radiation: Certain cancer drugs like Melphalan, Procarbazine etc are linked to Myelodysplastic Syndrome
  • Exposure to certain chemicals: Chemicals like benzene, pesticides and tobacco smoke are linked to Myelodysplastic Syndrome
  • Exposure to heavy metals: like mercury and lead
  • Inherited conditions: Some conditions like Down Syndrome, Bloom Syndrome, Fanconi Anaemia could increase the chance
  • Blood diseases: Like Paroxysmal nocturnal haemoglobinuria and congenial neutropenia

Types of Myelodysplastic Syndrome

The type of MDS is classified based on the number of blasts (immature cells) present in the blood and bone marrow, and how normal they are.

  • Refractory anaemia (RA)
  • Refractory anaemia with ringed sideroblasts (RARS)
  • Refractory cytopenia with multilineage dysplasia (RCMD)
  • Refractory cytopenia with multilineage dysplasia and ringed sideroblasts (RCMD-RS)
  • Refractory anaemia with excess blasts (RAEB)
  • Myelodysplastic syndrome, unclassified (MDS-U)
  • MDS associated with isolated del(5q)
  • Chronic myelomonocytic leukaemia (CMML) and juvenile myelomonocytic leukaemia (JMML)



Before Procedure

  • Physical examination
  • Mention all your medical history to your doctor including allergies
  • There will be tests and examination to assess your health 
  • You will be asked to stop taking certain medicines
  • Specific diagnostic tests will also be performed
  • Harvesting of stem cells from a matching donor
  • A small tube called central venous catheter will be inserted in a large vein to get IV medicines or blood transfusions

During Procedure

  • Your doctor prepares your body to receive the treatment called “preparative or conditioning regimen” wherein doses of chemotherapy will be given to you which could be ablative treatment or reduced intensity treatment
  • Radiation may also be done to prepare for transplantation
  • The immune system is weakened during this time and medicines are administered accordingly
  • This preparative regimen will last about a week
  • After conditioning, stem cell transplantation will be done on another day
  • The transplant may take 1 to 5 hours
  • It is not painful, and you will be awake throughout
  • Stem cells will be placed in bags, intravenously administered through the central venous catheter to the blood and bone marrow
  • You will get antibiotics to prevent or treat infections
  • Your blood will be tested often to check for red blood cells, white blood cells and, platelets
  • Re-growth or engraftment of stem cells will take a few weeks.  

After Procedure

  • You will need to stay for a few weeks for the new stem cells to be produced
  • You will have regular blood transfusions and platelet transfusions
  • You may suffer from side effects of the drugs
  • You will be given fluids by mouth or through a tube from your nose
  • Your blood count and other vital signs will be closely monitored
  • Medication will be given to treat infections or GVHD (graft-versus-host-disease)
  • You stay in the hospital for a month to three but could stretch depending on your whether your recovery.


Risks or Complications

Certain factors that could arise are:

  • Infections
  • GVHD (graft-versus-host-disease)
  • Acute myeloid leukaemia (AML) 
  • Bleeding
  • Clotting
  • Problem in the heart, kidney, or liver
  • Anaemia
