More About Coronary Angiogram

Coronary is a procedure that uses X-Ray images to assess if your coronary arteries are narrowed or blocked along with the extent and location of the affected areas. In Coronary Angiogram, a contrast dye is injected through a catheter inside the blood vessels and examines how the blood flows through the heart on an X-Ray screen. 

Coronary arteries supply the heart with blood. However, the supply can be disrupted in the blood vessels due to plaque which is made up of calcium, cholesterol, fat, and other substances. When a plaque is built up, serious coronary artery diseases such as heart attack could occur.

Coronary Angiogram is also known as Cardiac Angiogram, Catheter Arteriography, or Cardiac Catheterization. 

 

Indications of Coronary Angiogram

Coronary Angiogram can help diagnose heart conditions, plan future treatment, and carry out certain procedures. Indications are:

  • If there are symptoms of coronary artery disease like chest pain (angina)
  • Unstable angina
  • A heart disease you were born with (congenital heart disease)
  • Abnormal results on a non-invasive heart stress test
  • Other blood vessels problem or chest injury
  • You have valvular heart disease
  • You are diagnosed as having heart attack
  • You have aortic stenosis

 

Diagnosis and tests

  • Electrocardiogram (ECG)
  • Stress test
  • Chest X-Ray
  • Blood tests for cholesterol, triglycerides, and blood sugar
  • Cardiac catheterization

 

Procedure

Before Procedure

  • Thorough physical examination including blood pressure check and pulse
  • Your doctor will assess all your medical history
  • Diagnostic tests to be performed
  • You should tell your doctor if you have diabetes, pregnant, or had an allergic reaction to dye before
  • Certain medications may be stopped for a while
  • You must not eat or drink 8 – 12 hours before the procedure
  • You will need to empty your bladder just before an angiogram
  • The hair on your arm or groin may be shaved
  • Do not wear any kind of contact lenses, jewellery, or make-up.

During Procedure

  • This procedure takes about an hour 
  • An IV line will be inserted into the vein of your arm
  • You will be given local anaesthesia to help you relax along with medications, and fluids 
  • Electrodes will be used to monitor your chest during the surgery
  • Blood pressure cuffs and pulse oximeter will be used to check your pulses and amount of oxygen in your blood
  • Either from you groin or arm, a small thin tube called catheter is inserted in your blood vessel until it reaches the heart and arteries
  • A special dye called contrast image is injected inside the catheter and X-Ray images are taken
  • Your doctor will identify blockages
  • The catheter is removed, the incision closed with a clamp or small plug.

After Procedure

  • You will be under observation and monitoring for some time and lie flat to avoid bleeding 
  • You may be able to go home on the same day or stay in the hospital for a night
  • The doctor will evaluate the procedure and recommend the best treatment to open the arteries
  • You will need to drink a lot of water to flush out the dye from your body
  • You may eat if you want to eat something
  • You may experience bruising or discomfort for some days
  • Avoid lifting and strenuous exercises for some time
  • Do not take a tub bath or be in a pool for a few days, only shower is recommended
  • You will need to have a follow-up with your doctor
  • Do not drink or smoke at home.

 

Risks and complications

Coronary Angiogram is usually very safe but as with any procedure, may not be without complications such as:

  • Being allergic to the contrast dye
  • Bleeding on the site where the catheter was inserted
  • A small risk of damage to artery in the arm or leg, kidney damage, heart attack, or stroke.
  • Blood clots
  • Low blood pressure
  • Irregular heart-beat
  • Infections
