More About Augmentation Cystoplasty

Augmentation Cystoplasty also called Bladder Augmentation is a surgical procedure to enlarge the size of the bladder due to having too small a bladder or too high a pressure of urine to store urine normally. This causes incontinence. Surgery expands the capacity of the bladder and lowers the pressure within the bladder. 

When urine in the bladder cannot be properly drained or high pressure pushes the urine back to the kidney, it could cause incontinence, urinary tract infection (UTI) or kidney damage. 

Augmentation Cystoplasty is considered for serious incontinence only after other medical treatments have failed.

Causes for Augmentation Cystoplasty

Neuropathic and non-neuropathic causes contribute to severe bladder dysfunction. This can affect both children and adults. Neuropathic causes include:

  • Multiple sclerosis
  • Spinal cord injury
  • Tethered spinal cord
  • Myelodysplasia

Non-neuropathic causes are:

  • Classic or cloacal exstrophy
  • Radiation cystitis
  • Interstitial cystitis
  • Chronic cystitis including TB and schistosomiasis 
  • Detrusor instability
  • Defunctionalized bladder in a patient on dialysis



  • Incontinence
  • Recurrent UTIs
  • Pyelonephritis 
  • Progressive renal insufficiency
  • Impaired renal growth
  • Vesicoureteral reflux
  • Enuresis 
  • Debilitating urgency

Indications of Augmentation Cystoplasty

  • A marked reduction in bladder capacity or compliance
  • Despite medical interventions, symptoms still impair patient’s lifestyle
  • When the upper urinary tracts are at risk due to pressure



  • Patients who are unable or unwilling to perform a life-long intermittent catheterization. 
  • Patients with inflammatory bowel disease, bladder tumors, severe radiation cystitis, or severe renal insufficiency.
  • Poor surgical candidates and short life expectancy.

Diagnosis and tests

  • A thorough physical examination 
  • Blood and Urine tests
  • Bladder pressure studies to see how well the bladder holds urine or how full it is before leaking or pushes back to the kidneys
  • Examination of the bladder by means of Cystoscopy or Radio imaging
  • Renal and Bladder ultrasound
  • X-Rays

Types of Augmentation Cystoplasty

  • Open surgery: Through incisions, a part of the stomach or intestine is cut and attached to the opening in the bladder. A stoma may be created. 
  • Laparoscopy: Through small keyhole incisions in the abdomen, a laparoscope is inserted to have a clear view of the abdomen. A piece of the stomach or intestine is removed and attached to the bladder.


Before Procedure

  • Complete physical and medical examination
  • Discussion with your doctor on your condition and the type of surgery needed 
  • Eat low-fibre diet
  • Anti-biotics will be given to you
  • You will be given special liquids to cleanse your bowel causing loose stool
  • Strong laxatives may also be given
  • Surgery will be performed two days after being hospitalised

During Procedure

  • You will be given general anaesthesia
  • Depending on the surgery, it will take 4 – 8 hours
  • For Open surgery, incisions may be made on the stomach and top of the bladder respectively
  • A piece of tissue from your stomach or intestine is cut and attached to the top of the bladder as a patch, enlarging the bladder
  • For Laparoscopic surgery, key hole incisions are made on the abdomen and a laparoscope is inserted to visualise the abdomen clearly
  • The top of the bladder is then cut, a patch of tissue from the stomach or intestine removed and attached to the top of the bladder
  • The incisions are also stitched up
  • For both surgeries, a Stoma – an opening to insert the catheter into the bladder may be created.

After Procedure

  • You will need to stay in the hospital for up to 10 days for Open surgery or perhaps more, depending on your recovery
  • For Laparoscopy, it will take around 3 – 5 days depending on your recovery 
  • You will have a catheter attached to drain urine from your bladder
  • Through IV, you will be given pain medications, fluids, and antibiotics 
  • A tube will be inserted from your nose to the stomach to drain any content. This will stay until your stomach and intestines can function normally again
  • Take deep breaths to clear your lungs
  • Asked to take a walk
  • You will be taught to insert a catheter.
  • You will also be taught to self-catheterise using a tube once a catheter is removed
  • You will be taught how to irrigate bladder to prevent mucous from blocking the catheter tube
  • You will be taught how to clean the bladder and prevent infection
  • Follow-up surgery will be scheduled
  • Follow instructions of your doctor closely for an enduring success
  • Special diet up to 3 months

  • Bladder rupture
  • Urinary incontinence which may require further surgery
  • Risk of kidney stones
  • Mucus in the urine
  • Allergic reaction to anaesthesia
